Health Care and Dental Benefits

I. Medical Benefits

Advancing Opportunities offers medical insurance for all eligible full-time employees. Coverage begins on the first day of the month immediately following sixty (60) days of employment. If an employee chooses not to enroll in health insurance coverage when first eligible, he/she must wait until the next annual enrollment period to sign up for this benefit. The agency offers three medical insurance plans through AmeriHealth: a high option, middle option and a low option. Here is a description of the plan's Prescription Plan. These benefits are available to employees who live in either New Jersey or Pennsylvania. The middle plan option includes AmeriFlex, a Health Reimbursement Account. Here is an AmeriFlex HRA claim form. Employees pay premiums for the three medical insurance plans through pretax payroll deductions. If an employee can show proof of other medical coverage, they may opt out of Advancing Opportunities' plan. They would receive an additional $100 per month in salary in lieu of medical coverage. The Medical Waiver form must be filled out to choose which medical coverage is requested or to opt out of the agency’s medical plans. Also, to enroll in one of the AmeriHealth plans, and employee must complete the AmeriHealth Enrollment Form. If an employee is opting out of coverage, the AmeriHealth Waiver of Coverage form must also be completed.

II. Dental Benefits

Advancing Opportunities offers dental insurance for all eligible full-time employees. Coverage begins on the first day of the month immediately following sixty (60) days of employment.
